Before a youth can join us on trips (Destination Unknown, Retreats, Etc.) a consent form must be completed. Consent forms are to be filled out by youth parents and turned into the youth office. They will be kept on file in the youth office and will go with us on any trips off campus. We renew the forms yearly in the fall. If you are new to the youth ministry of Pleasant View UMC, you will need to fill one out as soon as possible.
